Your entire life history of vaccinations is online. You can see if you need a booster, or get an original vaccination at the
Wisconsin Immunization Registry website.
You will need you Name, Birthdate and one of the three following;
-- 3rd ID is : Social Security Number, Your Wisconsin BadgerCare Number ( Medicaid) or Regular Insurance Member number.
The search will prodiuce a full list of what you have been immunized for and list what vaccinations you may need.
