For a number of reasons. First, he planned it meticulously, pointing towards a pro.
They reported that his gun jammed mid killing, he calmly cleared the jam and carried on undisturbed. This requires proficiency with guns and calm under high stress.
Finally, he is young and short, maybe 5'6 which would be typical of South American drug gangs.
Of course it's speculation and it may turn out to be a person enraged at having lost a loved one to a medical disaster that uhc denied.
