It could be as easy as someone, staff member or not, setting up, say, a Press Editors Meeting or something like that on their cell a couple weeks prior, and Goldberg was on that list.
So when a new list or forwarded invite was being built for the Houthi Attack Signal Chat - a list pulled off the Contacts on that particular phone - Goldberg's name could easily have been auto-filled on the invite.
Whomever forwarded the invite didn't check before hitting "send", and since the icon just showed JG, no one on the call thought to find out if he was the actual intended participant or not.

There, I found the problem and gave the most plausible solution. And I didn't charge a million dollars to do so ..
