3. I don't know that there were any policy changes made after it was signed into law

But there is at least one insurer, Aetna I think, that is leaving New York. Some businesses have chosen to go the route of vouchers so that employees can choose which company they want. Other than those two, I've not heard of the law actually affecting anyone else's insurance. Unless you include changes the insurers are making to their own plans .
