Right hand/left hand not communicating on their side. I have a ton of that in my company. Usually, this occurs because people go through the motions doing task work instead of starting with a measurable outcome and then figuring out how to meet it best. I constantly fight the battle where people think that things will magically work because they put something online or in a cloud. Technology is a tool, not a solution.
