I wanted to elaborate more in my OP, but I was already tempting tldr. I am referring to your first two paragraphs.

At McD's we would not only do the testing but have the systems in place; the systems would have been tested and re-tested. Not only that, but because McD's is made up of franchisees, all of whom are independent businessmen who respect and trust each other for just that reason, they would be given a forum on which to report their experience. Such forums were (back at the time I was there) large owner-operator meetings.
