57. Immediately before the tragedy, the shooter was apparently video recording with his phone.

I think he was distracted by his act of video recording. He was not "in the moment" so to speak. A well trained officer would have been focusing on the situation and discerning between what is an imminent threat and what is not.

We will never know the purpose of the agent's video recording. For all we know, he may have been live streaming on Facebook in order to impress his friends.
