This rapid weight loss causes a reduction in subcutaneous fat — particularly noticeable in the face. The skin, especially in older adults or those with preexisting volume loss, may not bounce back, leading to a hollow or aged appearance.

Facial skin will sag and wrinkle as we age. Partly due to collagen loss as we age, and partly due to reduction in subcutaneous fat under the skin as weight goes down. Seems with those weight loss drugs, working quickly, the sagging gets pretty noticeable as it happens fast.

I noticed his face sagging a couple years ago and did some reading. Then all the sudden, it seems his face wasn't as saggy. Wondered if he stopped taking the drug, but my newer research pointed to people using facial injections to pump up the sagging the weight loss drugs causes in many.

As a former tailor, I noticed how loose his suits were the first time I noticed his saggy face. Weight loss so sudden his tailor couldn't keep up.
